Incident Reporting Intake Form
Bakersfield College is concerned about the safety and well-being of all students, faculty, and staff. In an effort to open the lines of communication across campus, the Office of Student Life has created this incident report form. If you were involved in a non-academic conduct incident, complete and submit this form. Your report will be sent to the Office of Student Life for further review.

Academic integrity is essential to the success of the College's educational missions, and violations of academic integrity constitute serious offenses against the entire academic community and the BC Standards of Student Conduct.
Student Rights and Grievances Intake Form
The Student Rights and Grievances procedures are established so that students can resolve difficulties/problems they encounter in college-related activities. The Student Rights and Grievances procedure is designed to consider an alleged wrong against a student (see KCCD Administrative Procedures 5530 for Student Rights and Grievances). Efforts will be made to resolve a grievance in a timely and fair manner. Students who contend they have been treated unfairly have the right, without fear of reprisal, to use a written procedure in their attempt to right an alleged wrong. Student grievances are taken seriously; therefore, the grievance must be of a compelling, substantive, and verifiable nature. Per KCCD AP 5530, BEFORE submitting this intake form to the Office of Student Life, the student should make a reasonable attempt to resolve the matter on an informal basis prior to requesting a grievance hearing and should attempt to resolve their grievance with the person whom the student has the grievance, that person's immediate supervisor, or the college administration. Once the student has spoken to either individual and has not received the desired resolution, the Dean of Students will assist the student in the process with this completed form. This form is only an intake form to start a conversation with the parties involved. Only one employee may be named per the intake form. Submitting this form may not directly start the Student Rights and Grievances procedures until it has been signed by the submitter. Repeated filings of the same grievance, filings of a frivolous nature, or capricious grievances against school personnel will be considered abuse of the student conduct and/or grievance procedures.
Bias Incident Reporting Intake Form
Bakersfield College is committed to provide safe spaces and to support a comprehensive reporting mechanism for incidents of bias so that BC may better understand the campus climate and work continuously to improve it. In all instances, individuals who experience acts of hate, bias, harassment, or discrimination should feel they have a safe space in which to report the behavior and receive support. Even if a reported incident is not one that constitutes a hate crime or an incident that violates the Standards of Student Conduct, the Office of Student Life learns from the reports and obtains feedback on how BC can improve some aspect of the campus climate. Reports may result in individual education or new efforts to improve some aspect of campus life.
Sexual Misconduct Reporting Intake Form
This form should be used to report concerns pertaining to or possible violations of the KCCD Equal Employment Opportunity and Unlawful Discrimination Policy (KCCD Board Policy 7D) such as sexual misconduct, sexual harassment, or sexual intimidation. Formal Complaint Under Title IX
Title IX is a law that intends to protect people from discrimination based on sex in education programs or activities that receive Federal financial assistance. The law requires that a complainant who is alleging sexual harassment submit a formal complaint in order to initiate the grievance process.
